Keratopigmentation: the complete guide

Keratopigmentation changes the apparent color of the eyes with a pigment placed in the cornea. This guide brings together what you need to know before deciding: what the procedure is, who it may suit, how it works, its risks, the results to expect and its price. Medical content: Dr Yoni Gozlan, ophthalmic surgeon in Paris.

What is keratopigmentation?

Keratopigmentation is surgery of the cornea, the clear front part of the eye. With the FLAAK technique (Femtosecond Laser-Assisted Annular Keratopigmentation), a femtosecond laser creates a very thin circular tunnel within the periphery of the cornea. A biocompatible medical pigment is placed in it: it visually covers the iris and gives the eyes a new color.

The central part of the cornea, the part you see through, is left clear. The procedure does not enter the eye: it touches neither the iris nor the lens, unlike iris implants. To compare with other methods: keratopigmentation or iris implant and keratopigmentation and colored contact lenses.

Who is it for?

Keratopigmentation is for adults (18 or older) who wish to change the color of their eyes for the long term, after careful thought. It may also be discussed in some medical situations (keratopigmentation and eye conditions).

It is not offered in some cases, including:

  • certain eye diseases, such as active glaucoma or severe keratoconus;
  • a current eye infection or inflammation;
  • pregnancy and breastfeeding;
  • a weakened immune system.

The eligibility test gives a first indication in 2 minutes. The final decision is made at the complete eye examination before the procedure. In detail: contraindications to keratopigmentation.

How does keratopigmentation work?

  1. First contact. You write to me on WhatsApp. We discuss your plans, your medical history and your expectations, and you can send me photos of your eyes.
  2. Choosing a date. Procedures take place during scheduled surgical sessions in Paris, announced in advance so you have time to plan your trip.
  3. Confirmation. Your date is confirmed with a €500 deposit, deducted from the price, or with proof of travel (plane or train ticket, or a hotel booking in Paris).
  4. The day of the procedure. A complete eye examination is carried out before the procedure. It is an outpatient procedure under local anesthesia with eye drops, lasting 15 to 20 minutes per eye, at the Centre Laser Victor Hugo, 27 bis avenue Victor Hugo, 75116 Paris.
  5. Afterwards. I give you your post-operative prescription. Temporary symptoms are common, so spending at least one night in Paris is recommended.

Risks and safety

Keratopigmentation is real surgery and carries risks. Temporary symptoms are common: in a survey of 42 patients operated on with the FLAAK technique (BMC Ophthalmology, 2023), 81% reported pain, 76% dry eyes, 71% tingling, 67% red eyes and 56% glare. All of these symptoms were temporary; in about half of the patients they had gone within 48 hours.

Rarer complications are possible: infection, inflammation, light sensitivity, or a color that is less even or lighter than planned, which may need a touch-up. The pigment cannot easily be removed, so the decision must be well thought through. The French National Academy of Medicine (June 2025) and the American Academy of Ophthalmology (January 2024) have published warnings about eye color-changing procedures.

The pre-operative examination is there to rule out eyes that should not be operated on. After the procedure, tell any ophthalmologist who examines you that you have had keratopigmentation. Further reading: risks, side effects and complications and complications and touch-ups.

Results

The color change is visible as soon as the procedure is over. In the first days the color often looks more intense; it may change slightly over the first weeks as the pigment settles into the cornea (why the color looks intense at first). The result is designed to be permanent; in some cases a touch-up may be needed, which is discussed at the consultation.

The shade is chosen with you to look natural. The color simulator lets you preview the shades, and the results page shows patients' before and after photos.

Price

The price is €5,500 for both eyes, whatever color you choose, including when the procedure is done in two stages. It includes the pre-operative consultation and complete eye examination, the femtosecond laser procedure and certified medical pigments. Keratopigmentation is a cosmetic procedure and is generally not covered by health insurance. All the details: keratopigmentation cost.

Could keratopigmentation be right for you?

Keratopigmentation: frequently asked questions

Is keratopigmentation painful?

The procedure is done under local anesthesia with eye drops: you mainly feel pressure. Pain or tingling is common in the hours and days that follow, then goes away.

How long does the procedure take?

15 to 20 minutes per eye, as an outpatient. You go home the same day; spending at least one night in Paris is recommended.

Is the result permanent?

Keratopigmentation is designed to be permanent. The shade may change slightly over the first weeks, and a touch-up is sometimes needed.

Can I go back to my original eye color?

The pigment placed in the cornea cannot easily be removed. That is why the decision must be well thought through and discussed at the consultation.

Does keratopigmentation damage eyesight?

The pigment is placed in the periphery of the cornea and the central area is left clear. Like any corneal surgery, it carries risks, including glare, which are explained at the consultation.

How much does keratopigmentation cost?

€5,500 for both eyes, whatever the color. Your date is confirmed with a €500 deposit, deducted from the price, or with proof of travel.

Where is the procedure performed?

At the Centre Laser Victor Hugo, 27 bis avenue Victor Hugo, 75116 Paris, during scheduled surgical sessions.
